YAC Game Day Interest Form

A chance to connect, cheer, and make memories together! YAC families, we are gathering interest in attending the Washington Commanders vs. Houston Texans game.

This opportunity is exclusively for current Youth Advisory Council members. Please complete the survey below to share your attendance interest and plans to use transportation arranged for the YAC group.

Tickets are limited, and YAC members have priority. Parents and guardians may express interest in volunteering as chaperones. Chaperone attendance is not guaranteed and depends on ticket availability and program needs.

Submitting this form does not reserve a ticket. Good Intentions Foundation will follow up with confirmation. Group transportation details, including departure and return times, will be shared with confirmed participants.
